The scanned copy of the original shall be provided during bidding (the original copy for reference);The bidder outside the customs territory shall provide valid certificate to prove valid business registration certificate.B.投標(biāo)人須提供制造商的放射性儀表《SIL2認(rèn)證證書(shū)》，且證書(shū)在有效期內(nèi)。投標(biāo)時(shí)需提供原件掃描件。The bidder must provide manufacturer's “ the SIL2 certificate of radioactive instrument”, and the certificate shall be within the validity period..The scanned copy of the original shall be provided during bidding. C.本次招標(biāo)不接受代理商投標(biāo)。投標(biāo)人應(yīng)是投標(biāo)貨物的制造廠商（包括制造廠商所在集團(tuán)內(nèi)負(fù)責(zé)銷(xiāo)售的公司）。如果投標(biāo)人是制造廠商所在集團(tuán)內(nèi)負(fù)責(zé)銷(xiāo)售的公司，投標(biāo)人應(yīng)提供聲明，說(shuō)明投標(biāo)人與制造廠商隸屬同一集團(tuán)，負(fù)責(zé)投標(biāo)貨物的銷(xiāo)售工作。Bid by Agent is not accepted.？The bidder shall be the manufacturer of bid goods, ？including？the sales company within the same group as the manufacturer of bid goods. If the bidder is the sales company within the same group as the manufacturer, the bidder should provide relevant documents to prove that the sales company is within the same group as the Manufacturer and it is responsible for the sales of the bid goods.D. 是否接受聯(lián)合體投標(biāo)：否Joint Venture: Not AcceptedE.未領(lǐng)購(gòu)招標(biāo)文件是否可以參加投標(biāo):不可以Bid without the bidding documents：NOT Allowed？（2）資質(zhì)要求Qualification requirement:（2.1）投標(biāo)人須提供投標(biāo)人的《輻射安全許可證》，且證書(shū)在有效期內(nèi)。投標(biāo)時(shí)需提供原件掃描件。The bidder must provide its “Radiation Safety Permit”,and the certificate shall be within the validity period..The scanned copy of the original shall be provided during bidding.（2.2）投標(biāo)人須提供制造商的放射性儀表《防爆證書(shū)》，需體現(xiàn)氣體防爆認(rèn)證和粉塵防爆認(rèn)證，防爆等級(jí)不低于GB/T 3836或IEC-60079 標(biāo)準(zhǔn)規(guī)定的Ex db IIC T6 Gb+Ex tb IIIC T135℃ Db，防護(hù)等級(jí)不低于IP66，且證書(shū)在有效期內(nèi)。投標(biāo)時(shí)需提供原件掃描件。The bidder must provide manufacturer's “Explosion Proof Certificate” for the radioactive instruments,and meet the Ex db IIC T6 Gb+Ex tb IIC T135℃ Db explosion-proof and IP66 protection requirements as specified in the GB/T 3836 or IEC-60079 standards, and the certificate shall be within the validity period. The scanned copy of the original shall be provided during bidding.？（3）業(yè)績(jī)要求Requirement for Bidder's reference:2015年1月1日至投標(biāo)截止日（以合同簽署時(shí)間為準(zhǔn)），投標(biāo)人所投產(chǎn)品的制造商應(yīng)具有2個(gè)項(xiàng)目的聚烯烴裝置放射性儀表業(yè)績(jī)，且單個(gè)合同放射性儀表數(shù)量大于或等于15臺(tái)（同一項(xiàng)目提供多個(gè)合同視為一個(gè)業(yè)績(jī)）。聚烯烴裝置包括聚丙烯裝置、低密度聚乙烯裝置、高密度聚乙烯裝置、全密度聚乙烯裝置（提供的業(yè)績(jī)滿足其中一個(gè)裝置即可）。如果投標(biāo)人為制造商所在集團(tuán)內(nèi)負(fù)責(zé)銷(xiāo)售的公司，應(yīng)提供該制造商的業(yè)績(jī)。From January 1st, 2015？to the deadline for submission of the bid (subject to the signing time of the contract), the manufacturer of the products supplied？by the bidder shall have？at least 2？project？Radioactive Instruments？for？Polyolefin Plants？？performance, and the number of Radioactive Instruments of single contract need to be greater than or equal to 15 sets(Providing multiple contracts for the same project is considered as one performance).？Polyolefin plants include polypropylene plant, low-density polyethylene plant, high-density polyethylene plant, full-density polyethylene plant. (The performance provided meets only？one of the plants).If the bidder is a company responsible for sales within the manufacturer's group, the performance of the manufacturer shall be provided.投標(biāo)人須按規(guī)定格式提交業(yè)績(jī)表，并提交相關(guān)業(yè)績(jī)證明文件。業(yè)績(jī)證明文件包括但不限于：銷(xiāo)售合同復(fù)印件和用戶簽字或蓋章的到貨驗(yàn)收材料。投標(biāo)人所提交的業(yè)績(jī)證明文件必須至少體現(xiàn)以下內(nèi)容：合同簽署時(shí)間、合同簽署頁(yè)（國(guó)內(nèi)貿(mào)易合同應(yīng)有雙方蓋章，國(guó)際貿(mào)易合同應(yīng)有雙方簽字或蓋章）、項(xiàng)目名稱、制造商名稱、裝置名稱、貨物名稱、貨物數(shù)量。若業(yè)績(jī)合同為年度協(xié)議，除提供年度協(xié)議外，還應(yīng)提供相應(yīng)的已到貨訂單，訂單內(nèi)容或編號(hào)應(yīng)與年度協(xié)議相關(guān)聯(lián)。同一個(gè)年度協(xié)議下提供1個(gè)或以上的訂單及與訂單對(duì)應(yīng)的到貨驗(yàn)收材料均算為1個(gè)有效業(yè)績(jī)。The Bidder shall furnish reference list in given form, and provide corresponding documentary evidence. The documentary evidence shall include but not limited to: copies of sales contract and？acceptance evidence of goods delivery signed or sealed by the user.？The？reference evidence documents furnished by？Bidder shall as least demonstrate the content as follows: Date of signing contract, contract signing page(domestic trade contracts should have signatures or seals from both parties, international trade contracts should have signatures or seals from both parties), name of project,？name of manufacturer, name of the plant, name of goods,？number？of goods？.If the performance contract is an annual agreement, in addition to providing the annual agreement, corresponding received orders should also be provided, and the order content or number should be associated with the annual agreement. Providing one or more orders and corresponding acceptance evidence？under the same annual agreement is considered as one valid performance.未提交業(yè)績(jī)證明文件，或通過(guò)所提供的業(yè)績(jī)證明文件無(wú)法認(rèn)定滿足上述業(yè)績(jī)要求的，均視為無(wú)效業(yè)績(jī)。？If no documentary evidence furnished, or documentary evidence furnished can not demonstrate the above performance requirements, the relevant reference will be deemed invalid.是否接受聯(lián)合體投標(biāo):不接受未領(lǐng)購(gòu)招標(biāo)文件是否可以參加投標(biāo):不可以4、
